The Affordable Care Act, or Obamacare, was signed into law with the goals of providing quality and affordable health care to every American, but there is concern that not enough young adults are gaining health insurance. Some believe it is because of the "young invincible" mentality of being healthy enough to not need health insurance, and others claim that the cost of health care is the main reason behind low enrollment rates in young adults. However, young adults may not be obtaining insurance because of a lack of understanding and awareness concerning the ACA. Do young adults understand how the ACA functions, and does this understanding (or lack thereof) determine their opinions towards it? In order to research this question, students at Arizona State University were given the opportunity to complete a survey and interview detailing their knowledge of Obamacare and how they felt about the health care law. Results indicated that though many respondents supported the law, respondents did not feel like they had enough information to understand the health care law, affecting their knowledge of it. These findings imply that in order for the ACA to be considered successful among young adults, awareness and education of the law must increase in order for young people to feel like they have an adequate understanding of it.

As the complexity of healthcare continues to rise, the need for change in healthcare delivery is more prominent than ever. One strategy identified by the World Health Organization (WHO) for responding to these increasing complexities is the use of interprofessional practice and education to improve patient outcomes, reduce costs, and enhance the patient experience of care (Triple Aim). Interprofessional collaboration among diverse disciplines is evident on the Phoenix Biomedical Campus, integrating a wide variety of institutions and multiple health profession programs; and at the Student Health Outreach for Wellness (SHOW) free clinic, -- a successful tri-university, student-led, faculty mentored, and community-based model of interprofessional learning and care -- based in downtown Phoenix. This project conducted a comparative analysis of interprofessional components of 6 different clinical models in order to provide recommendations for best practice implementation. These models were chosen based on availability of research on interprofessionalism with their clinics. As a result, three recommendations were offered to the SHOW clinic for consideration in their efforts to improve both patient and educational outcomes. Each recommendation was intentionally formulated for its capacity to increase: interprofessionalism and collaboration between multiple disciplines pertaining to healthcare, among healthcare professionals to promote positive patient and educational outcomes. These recommendations include implementing an interprofessional education (IPE) course as a core component in an academic program's curriculum, offering faculty and professional development opportunities for faculty and mentors immersed in the interprofessional clinics, and utilization of simulation centers. Further studies will be needed to evaluate the impact these specific interventions, if adopted, on patient and educational outcomes.

In 2015, the World Health Organization cited antibiotic resistance as one of the greatest current challenges to global public health. A major driver of the evolution of antibiotic resistance is the overuse and misuse of these drugs. While antibiotic stewardship, education campaigns, and health policy attempt to limit drug use globally, public understanding of antibiotic resistance and its consequences are lacking. The goal of this study is to analyze the social and cultural influences of antibiotic knowledge and usage behavior. Over a three-month period, I interviewed 211 laypersons in Guatemala, Spain, the Netherlands, India, South Africa, and New Zealand to understand their ideas, perceptions, and behaviors regarding antibiotics and compared results across countries. While an overall consensus across countries does exist, I found significant differences between low and high income countries as well as between low and high antibiotic consumption countries. Additionally, I found that having increased public health knowledge is related to lower antibiotic "risky" behavior. These results help contextualize national data on antibiotic consumption and resistance by illustrating relationships between access, beliefs, and consumption patterns within populations. The results also inform the development of community and culture specific educational campaigns regarding antibiotic resistance.

Given the prominence of alcohol in the Korean adult's daily life, further investigation into the negative effects of harmful alcohol consumption and the treatment of and culture surrounding alcohol use disorders is needed. An investigation into the status and treatment of alcohol use disorders in South Korea was chosen due to the significant cultural differences from the United States with regards to alcohol consumption, mental health, and healthcare. The investigation used academic and grey literature, news reports, and current healthcare and public health policy to evaluate South Korea's weak areas in addressing alcohol use disorders. Greater investment into mental health research and healthcare delivery, as well as further development of the continuum of care to help patients transition from treatment of the acute symptoms of an alcohol use disorder to lifetime management are needed. Lastly, increased alcohol regulatory policies are recommended.

Homelessness is a pervasive in American society. The causes of homelessness are complex, but health and homelessness are inextricably linked. Student-run free clinics care for underserved populations, including people experiencing homelessness, but they have multiple agendas—to provide care but also to give students hands-on experience. It is plausible that these two agendas may compete and give patients sub-par quality of care.
This study examines patient care in the SHOW free clinic in Phoenix, Arizona, which serves adults experiencing homelessness. This study asks two questions: First, do clinicians in Phoenix’s SHOW free clinic discuss with patients how to pay for and where to access follow-up services and medications? Second, how do the backgrounds of patients, measured by scales based on the Gelberg-Anderson behavioral model for vulnerable populations, correlate with patient outcomes, including number of unmet needs in clinic, patient satisfaction with care, and patient perceived health status? To answer these questions, structured surveys were administered to SHOW clinic patients at the end of their visits. Results were analyzed using Pearson’s correlations and odds ratios. 21 patients completed the survey over four weeks in February-March 2017. We did not identify any statistically significant correlations between predisposing factors such as severity/duration of homelessness, mental health history, ethnicity, or LGBTQ status and quality of care outcomes. Twenty nine percent of surveyed patients reported having one or more unmet needs following their SHOW clinic visit suggesting an important area for future research. The results from this study indicate that measuring unmet needs is a feasible alternative to patient satisfaction surveys for assessing quality of care in student-run free clinics for homeless populations.

In an increasingly interconnected world, the 17 Sustainable Development Goals are the United Nations’ framework for ensuring we continue to transform our world for the better, leaving no population behind. This study examines how the terminology of Sustainable Development Goal 17 for global partnership affects its implementation, focusing on “building capacity”—a widely referenced target in the development arena—and the involvement of the private sector. Key informant interviews with experts in the fields of conflict of interest, ethics, and development revealed a wide variety of (often conflicting) notions about partnership, frameworks for capacity development, and the interactions between public and private actors. A literature review of key policy documents examined the terminology and implementation of multistakeholder partnerships, and analysis offered considerations for risks and suggestions in policy terminology. Results indicate a need for increased attention to the use of partnership terminology as a catch-all term to encompass development work, and makes several recommendations for changes to combat misuse of the partnership label. Finally, this study acknowledges that there is a continued need for research-based evidence for effectiveness of the partnership-based development approach.

Since underserved individuals do not have a steady supply of food, this study explored whether their standards of what they view as healthy differs from individuals who can afford a basic living that includes food and shelter. Data collection from surveys provided information to see whether the struggles of obtaining food affects what is perceived as healthy, and whether there is a difference in dietary habits, perception of body image, and self-esteem. Homeless individuals displayed that they were more aware than non-homeless individuals that the food they were consuming was unhealthy. They were also less satisfied with their daily food diet, as most of them wished that they ate greater quantities of certain foods. Their daily food intake did confirm that they consumed more unhealthy food that lacked nutrition compared to non-homeless individuals. They also generally believed that thicker body images were healthier and more attractive compared to non-homeless people who thought that thinner body images were healthier and attractive. Homeless people also generally ranked lower on the body image scale than the image they thought was most desirable and healthy. This revealed a lack of satisfaction with their own current body. Additionally, the self-efficacy score displayed that homeless individuals generally scored lower for their self-esteem level compared to non-homeless people. This demonstrated that their daily struggles and lifestyle impacts their emotions and overall confidence.

Under-representation of women doctors in medical work force despite their overwhelming majority in medical schools is an intriguing social issue for Pakistan raising important questions related to evolving gender relations in Pakistani society. Previous research on the broader issue of under-representation of women in science has focused primarily on the structural barriers to women’s advancement. It does not account for the underlying subtle (and changing) gendered power relations that permeate everyday life and which can constrain (or enable) the choices of women. It also does not address how women are not simply constructed as subjects within intersecting power relations, but actively construct meaning in relation to them. It raises interesting questions about the cultural shaping of subjectivities, identities and agency of women within the web of power relations in a society such as Pakistan.

To analyze the underlying dynamics of this issue, this dissertation empirically examines the individual, institutional and social factors which enable or affect the career choices of Pakistani women doctors. Based on the ethnographic data obtained from in-depth, person centered, open ended interviews with sixty women doctors and their families, as well as policy makers and the stake holders in medical education and health administration in Lahore, Pakistan this dissertation seeks to address the complex issues of empowerment and agency in the context of Pakistani women, both in individual and collective sense.

Participation in medical education is ostensibly an empowering act, but dissecting the social relations in which this decision takes place reveals that becoming a doctor actually enmeshes women further in the disciplinary relations within their families and society. Similarly, the medical workplaces of Pakistan are marked by entrenched gendered hierarchies constraining women’s access to resources and their progression through medical career. Finally, the political implications of defining work in medicine, and devaluing care in capitalist economies is explored.

A nonprofit organization’s ability to help its target population depends strongly on the collaboration of the organization’s staff and leadership. An organization that spans across international borders must overcome adversity, particularly communication and power inequity. The International Alliance for the Prevention of AIDS (IAPA) is a nonprofit with staff in the U.S. and India, making it an international partnership. This research evaluates to what extent the Indian partners believe IAPA meets Sustainable Development Goal 17: “to revitalize the global partnership for sustainable development.” I developed three semi-structured interview protocols for volunteers, employees, and IAPA beneficiaries. After interviews were conducted and transcribed, 5 major themes were identified from coding keywords. First, I grouped definitions of "success" in a partnership to create a baseline of expectations. Second, I assessed the extent of participants' knowledge about the U.S. role in IAPA. Third, I identified areas of strength. Fourth, I identified areas of improvement and grievances. Fifth, I assessed the Indian partners' views on mutualism within IAPA. Results indicated that participants believed communication, cooperation, and respect were traits of a successful partnership. The participants believe IAPA mostly exhibit these values, but that the U.S. role as a decision maker can hinder these. They desire more transparency but overall believe IAPA is beneficial and mutualistic. These findings can be furthered by assessing U.S. staff and board member perceptions of the partnership. By continuously investigating the state of international partnerships, we can learn more about how to create sustainable models for the future.
