Purpose: To implement a non-pharmacologic approach to reduce anxiety in patients receiving radiation treatment.

Background and Significance: Anxiety is a prevalent health problem affecting up to 30% of the general population. Over 95% of radiation oncology patients report anxiety often related to not knowing what to expect during treatments. The current guideline for management of anxiety is the use of benzodiazepines; which can impose a risk to patients. Evidence demonstrates virtual reality videos can be an effective non-pharmacologic approach to reduce anxiety..

Design: Observational cohort evidence based project.

Setting: Outpatient radiation oncology facility in the Southwest United States.

Sample: 40 patients who have not previously undergone radiation therapy, but have been prescribed radiation treatments for malignant or non malignant conditions.

Methods: Patients who had not previously undergone radiation treatments were identified based on chart review and consented for participation in the project. The patient's were assessed for both their general (trait) anxiety and situational (state) anxiety using The State Trait Anxiety inventory (STAI). They were then shown a virtual reality video that walked them through what they would experience during their radiation treatment. The patient's were re-administered the STAI questionnaire following the video to assess any change in anxiety levels.

Variables: Trait and State anxiety scores before and after the virtual reality video intervention.

Findings/Outcomes: A paired t-test was conducted to assess the data. There was a significant difference in the state anxiety scores pre intervention (m = 32.65, SD = 11.47) and post intervention (M = 25.50, SD 8.95), p =< 0.001.

Conclusions: The use of virtual reality videos was an effective non-pharmacological approach to reduce anxiety in patients receiving radiation treatment.

Implications for nursing: The results provide support for the use of a virtual reality intervention as a non-pharmacologic option to reduce anxiety for patients.

Aims: The goals of this project were (1) develop a sepsis clinical guideline, (2) enhance direct patient care staff knowledge of sepsis and (3) survey staff comfort level with identifying sepsis post intervention.

Background: Sepsis remains a significant healthcare problem associated with high treatment costs and high mortality rates. Older adults are at an increased risk for developing sepsis, especially when age is combined with any type of compromising factor, such as chronic illness, recent hospitalizations, wounds, or invasive devices. Current evidence demonstrates that sepsis screening is effective for early identification of sepsis. Early identification of sepsis improves time to treatment initiation, which improves outcomes.

Methods: An evidence-based, provider approved clinical guideline was developed for a post-acute care facility after an extensive review of the literature. Upon implementation, brief educational sessions were provided to direct patient care staff. Participants completed pre- and post-tests as well as a demographic survey. A satisfaction survey was administered 30 days post intervention. A paired samples t-test was used to analyze the difference in test scores. Pearson's correlation was used to analyze the relationship between staff comfort levels and the clinical guideline.

Results: The samples included 25 participants in the educational intervention and 18 in the satisfaction survey. There was a significant difference in the scores between pre-test (M = 72.3, SD = 12.43) and post-test scores (M = 86.6, SD = 10.2); t(24) = -5.578, p < 0.001. There was a significant correlation between staff who felt comfortable in identifying sepsis with ease of screening (r = .831, p < .01) and high comfort levels with the policy (r = .889, p < .01).

Conclusion: Utilizing a clinical guideline, coupled with education, improves staff knowledge and comfort identifying sepsis in the post-acute care setting, which may improve early recognition and treatment initiation. This outcome is clinically significant as patients in this setting represent a vulnerable population.

Background: The purpose of this project was a culture assessment on nurses’ perception about evidence-based practice (EBP) prior to creating an EBP training program. EBP improves patient outcomes, job satisfaction and retention, and decrease healthcare costs. Methods: A descriptive study design was used. Nurses at a hospital and outpatient cancer center were sent a voluntary anonymous survey through work email. 630 nurses were invited to participate, and 245 completed the survey with a response rate of 38.9%. The survey consisted of 3 instruments developed by Melnyk, along with demographic information. Results: Most nurses surveyed answered “strongly agree” or “agree” to questions pertaining to EBP knowledge and implementation. “I believe that EBP results in the best clinical care for patients,” resulted in 93.9% answering “strongly agree” or “agree.” For questions referring to the organizational culture towards EBP, the results were less positive. Only 59.6% answered, “strongly agree” or “agree” to “My organization provides EBP mentors to assist clinicians in implementing EBP.” Discussion: Strengths and current EBP beliefs and practices can be identified through organizational assessment. This project will spur further discussion and knowledge growth of EBP practice, inspiring the nursing organization to examine current culture to create an innovative community. Conclusions: Along with areas of strength including a positive knowledge and use of EBP in the organization, barriers to implementation such as leadership support were identified. These factors will influence further development of mentors and education and the formulation and implementation of the fellowship program.

Background and Objectives: Electronic cigarette use, known as vaping, among adolescents was declared a public health epidemic in 2018 and has acute and chronic health consequences. Healthcare providers (HCP) play a pivotal role as health-related educators, including counseling against vaping. Primary HCPs report a lack of adequate knowledge, confidence, and screening for adolescent vaping. Increasing HCP’s vaping awareness and knowledge may increase rates of adolescent vaping prevention screening and counseling. Rosswurm & Larrabee Model and Health Belief Model were utilized in project design and implementation. Methods: Primary HCPs (n = 8) that provide care to adolescents at a pediatric clinic in Phoenix, Arizona completed online pre- and post- education surveys measuring vaping knowledge before and after viewing an evidence based online educational video. Participation was voluntary, open to all clinic HCPs, and informed consent was provided before the intervention. Data analysis was completed with Intellectus Statistics using descriptive and inferential statistics. Results: Results of the paired samples t-test was significant based on "?=.05" , t(7) = -3.56, p = .009. The mean of the post-education survey (12.38) was significantly higher than the mean of the pre-education survey (9.62). Descriptive statistics found 85.71% of HCPs reported increased intent to counsel for vaping and 57.14% of HCPs reported increased implemented vaping counseling with their adolescent patients four-weeks post intervention. Conclusions: HCP vaping knowledge rates and vaping-related counseling and surveillance significantly increased after viewing the educational video. Implementing mandatory HCP vaping education training could increase adolescent vaping prevention interventions and counseling within primary care settings.
