Virtual Patient Simulations (VPS) are web-based exercises involving simulated patients in virtual environments. This study investigates the utility of VPS for increasing medical student clinical reasoning skills, collaboration, and engagement. Many studies indicate that VPS provide medical students with essential practice in clinical decision making before they encounter real life patients. The utility of a recursive, inductive VPS for increasing clinical decision-making skills, collaboration, or engagement is unknown. Following a design-based methodology, VPS were implemented in two phases with two different cohorts of first year medical students: spring and fall of 2013. Participants were 108 medical students and six of their clinical faculty tutors. Students collaborated in teams of three to complete a series of virtual patient cases, submitting a ballpark diagnosis at the conclusion of each session. Student participants subsequently completed an electronic, 28-item Exit Survey. Finally, students participated in a randomized controlled trial comparing traditional (tutor-led) and VPS case instruction methods. This sequence of activities rendered quantitative and qualitative data that were triangulated during data analysis to increase the validity of findings. After practicing through four VPS cases, student triad teams selected accurate ballpark diagnosis 92 percent of the time. Pre-post test results revealed that PPT was significantly more effective than VPS after 20 minutes of instruction. PPT instruction resulted in significantly higher learning gains, but both modalities supported significant learning gains in clinical reasoning. Students collaborated well and held rich clinical discussions; the central phenomenon that emerged was "synthesizing evidence inductively to make clinical decisions." Using an inductive process, student teams collaborated to analyze patient data, and in nearly all instances successfully solved the case, while remaining cognitively engaged. This is the first design-based study regarding virtual patient simulation, reporting iterative phases of implementation and design improvement, culminating in local theories (petite generalizations) about VPS design. A thick, rich description of environment, process, and findings may benefit other researchers and institutions in designing and implementing effective VPS.

Extensive analysis, primarily based upon online market research and personal correspondence with the client, was conducted for this report. Detailed within this paper are several areas where the client can significantly lower costs and increase future revenues by modifying practices employed by the managing company. From the analysis provided, the client has the opportunity to create and grow a well-organized, profitable business with a sustainable future.

This action research study investigates the outcomes and barriers to persistence for adult learners in a medical assisting program at a private Catholic college in central Iowa. The research compares the impact of a work-based learning track and a traditional learning track to understand how the differences in the learning track support adult learners' andragogical preferences, reduce barriers, and helps students graduate, persist, and pass their licensure exam.A review of the existing scholarly, theoretical, and empirical research suggests several optimal ways to deliver adult education and many alternative learning models designed to support adult learners’ unique needs and life experiences. Andragogical principles help explain the differences between adult learners versus children and their preferences and priorities that shape their orientation toward learning. These principles, combined with the experiential learning theory, offer theoretical support for developing alternative learning paths such as work-based learning. Utilizing a mixed-method action research design, 51 current students, graduates, and stop-outs provided feedback through a survey, and four individuals from the same respondent group engaged in a virtual interview. Students in the work-based learning track reported feeling better prepared for their licensure exam, had a more positive overall experience, and found the program less challenging compared to their counterparts in the traditional pathway. Additionally, institutional data was evaluated and adults in the work-based learning track had higher rates of persistence, graduation, and licensure pass rates. The study illuminates that combining education with on-the-job training in the form of work-based learning has significant benefits for adult learners when compared to a traditional learning track that dominates much of modern academia. The research findings highlight the importance of embracing andragogical principles when designing adult education specifically, their intrinsic motivation and orientation to learning. This is a strength of a work-based learning model as students can reinforce classroom learning with hands-on training, facilitating the cycle of learning through concrete experiences and active experimentation. The medical assisting work-based learning track also alleviates common challenges to student persistence, such as tuition costs, by involving employers in financing students' education in exchange for a work commitment post-graduation.

The goal of this project was to create a quasi experimental study using an education module that teaches evidence-based practice methods. The theoretical frameworks used to create the educational content were the self-efficacy theory and the Health Belief Model. The evaluation methods used are based on the Kirkpatrick four level model. An education module was created to be culturally and regionally relevant to South Sudan and Malawi. The education module was designed to be part of the SolarSPELL Health: Nursing and Midwifery Library. This was done by performing a literature review, curating resources, creating the educational materials, creating learning scenarios, curating relevant belief scales, and integrating the content into the SolarSPELL Health: Nursing and Midwifery Library. The on ground implementation of the materials was not a part of this project, but instead is planned for future research. This project creates a foundation from which SolarSPELL Health can implement the resources at a future date. In the long term, the goal of implementing the experiment is to improve maternal mental and physical health outcomes in South Sudan and Malawi, both of which have extremely high rates of maternal mortality and morbidity.

The SolarSPELL Health: Nursing and Midwifery library was created to provide health students and professionals in developing and underprivileged countries with resources for evidence based practice learning, and to help improve women’s health in the country with the highest maternal mortality rate in the world. Our team partnered with local health schools to identify the greatest areas of need. The SolarSPELL team searched for open access materials aimed towards students and professionals in the health field, ensured that this content was relevant to the location and took into account the resources available, curated this content and ensured consistency throughout the process, and lastly packaged the material in a platform that was easy to access and navigate. South Sudan is the newest country in the world, and they lack significant resources, as do all of the countries that SolarSPELL serves. Receiving this library will allow the health students and professionals in South Sudan to utilize the resources they currently have, to provide evidenced based care, both saving and improving lives.
