The trained singer utilizes an awareness of her body as an instrument. When she becomes pregnant, her body changes in numerous ways to support the pregnancy. Many of these changes have great impact on her ability to sing during the pregnancy and postpartum periods. The voice may be altered positively or negatively by the release of hormones. The body undergoes many changes that affect the posture and breathing required for singing. Most notably, the abdominal muscles are greatly impacted by the pregnancy. They are stretched by the growing uterus, and this affects their function. In addition, the linea alba (the connective tissue between the halves of the rectus abdominis) is softened by hormonal increases and subject to stretching as the uterus grows, predisposing it to weakness. Since the other abdominal muscles attach to the linea alba via connective tissue, maintaining the integrity of the linea alba during pregnancy and postpartum is vital to the operational function of the abdominal muscles. Protecting the vulnerable linea alba must be deliberately undertaken in two parts. First, conscious exercise is needed to preserve the linea alba during pregnancy and to rehabilitate it after pregnancy. Targeted exercises strengthen the transverse abdominis and shorten and approximate the two halves of the rectus abdominis. Second, modifications in daily movement are necessary to protect the linea alba while performing routine activities. Cesarean sections present additional surgical concerns for singers, including abdominal incisions, use of medication, and the rare need for general anesthesia via intubation. Recovery from a cesarean can be difficult due to abdominal pain, yet steps may be taken to speed healing at the hospital and at home. This paper provides an overview of how pregnancy affects the singer, discusses the effects of pregnancy and cesarean section, and provides a plan to protect the abdominal muscles during pregnancy and rehabilitate them in the postpartum period. It combines information from the fields of physical therapy, medicine, and surgery into a guide for the singer and voice teacher.

According to traditional Chinese medicine, the month following childbirth is an important period marked by an imbalance of two opposing forces that together make up one’s health and wellbeing. A set of specialized practices called zuoyuezi (sitting the month) aid both the woman’s recovery and restoration of the balance, and require the help of someone else, usually the woman’s mother or mother-in-law. While studies conducted on the practice’s psychosocial and physical benefits have produced varied results, zuoyuezi continues to persist in Hong Kong, China, and Taiwan. Since the late twentieth century, professional zuoyuezi centers have become very popular as a commercial health care business. While the month experiences of Taiwanese and Chinese women have been widely studied, there is little research on physicians’ opinions regarding the practice, especially in Western medical settings. Taiwanese physicians, who have been trained in the Western medical tradition, present interesting case studies as both experts in Western medicine and citizens in traditional Taiwanese society. The purpose of this project is to observe how Taiwanese physicians negotiate primarily cultural practices with their professional training, and whether there is a conflict between physicians’ beliefs about zuoyuezi and physicians’ personal experiences with the practice. Twenty-seven semi-structured interviews of Taiwanese physicians were conducted at two sites in Taiwan regarding their perspective and understanding of zuoyuezi and their personal experiences with it. Following qualitative analysis, the findings showed that physicians used their Western medical training to explain the traditional worldview that holds zuoyuezi. Secondly, physicians acknowledged the benefits of zuoyuezi and the influence of culture as two primary factors in its continued existence. Finally, physicians incorporated zuoyuezi into their personal lives while modifying the traditional practices. Overall, Taiwanese physicians did not appear to have direct conflict with the cultural practice, zuoyuezi, using their medical expertise to rationalize its existence while becoming active participants and co-creators in the practice.

Many stressors today are psychological rather than physical and are influenced by the brain's perception of the stressor. The peripartum period is a particularly volatile time that is susceptible to new and stronger stressors. This current study investigates the relationship between self-reported perceived stress levels and physiological cortisol reactivity levels in new mothers at the 12-week postpartum time point. In addition, it examines the relationship between the mother and infants' physiological cortisol reactivity levels at 12-weeks postpartum. This current study is part of a longitudinal study and assessed these two correlations for 181 mother-infant dyads from a low income Mexican American population. The self-reported stress levels were assessed using the 4-item Perceived Stress Scale (PSS) and the cortisol reactivity data was gathered using four salivary cortisol samples taken from both mother and infant surrounding 5 interaction tasks and analyzed using Area Under the Curve with respect to ground (AUCg). Unexpectedly, the results found no correlation between perceived and physiological stress levels in the mothers, with a Pearson correlation of 0.114 and a p-value of 0.129. However, there was a positive correlation between mother and infant cortisol reactivity, with a correlation of 0.632 and a p-value less than 0.0001. This early postpartum period plays a significant role in developing HPA axis regulation for infants and developing productive mother-infant interactions. The physiological and psychological risks of chronically elevated stress for both mothers and children were addressed in this study as well, with implications for means to address and mitigate potential cortisol dysregulation.

The microorganisms that form the gut microbiome begin colonizing the gut at birth. The microorganisms will establish a symbiotic relationship with the host when in a state of eubiosis. However, when there is an imbalance or lack of diversity in the microorganisms present in the gut microbiome, it will shift to a state of dysbiosis, which increases the risk of inflammatory bowel and neurodegenerative diseases. There are known factors that affect the infant intestinal microbiome: mode of delivery, gestational age, and exposures, such as the use of antibiotics and feeding methods. Studies have shown that maternal lifestyle, particularly stress levels and diet, has an association with the infant microbiome diversity. However, there is limited literature that examines how postpartum maternal sleep influences the intestinal microbiome diversity in infants. We hypothesized that sleep efficiency below 85%, total sleep time and nocturnal sleep time below 7 hours, and later nocturnal sleep onset would be associated with decreased gut microbiome diversity within the sample and between samples. 27 Mother-infant pairings were used for this sub-analysis. Postpartum maternal sleep data was obtained via actigraphs and sleep diaries while infant intestinal microbiome data was obtained through fecal samples swabbed from soiled diapers. Actigraphs recorded time, movement, temperature, and light for five 24-hour periods, and sleep diaries were used to supplement the actigraphy and ease the scoring of sleep. DNA samples were extracted and sequenced using next-generation sequences, and QIIME2 was used to analyze these sequences. Alpha- and beta-diversity results following sequencing revealed good postpartum maternal sleep efficiency is associated with increased infant gut microbiome diversity within and between samples. There was a trend for nocturnal sleep onset being associated with beta-diversity measurements which were driven by phylogenetic differences and abundance of bacteria. Nocturnal total sleep time and 24-hour total sleep time were not associated with alpha- and beta-diversity. Our findings as well as the scarcity of the literature illustrate the need for further investigations on the mechanics of maternal sleep efficiency association with infant gut microbiome. Future research is needed to further evaluate the trending nocturnal sleep onset influences on the infant gut microbiome beta-diversity with a larger and more diverse sample.
